Please be advised that the original hard copies students receive from the Department of Speech & Hearing Science at ASU overrule any on-line forms you may download, should there be a discrepancy between the official hard copies and the electronic version. Please make certain that the documents obtained are the same as the ones that were in effect at the time you enrolled in the program. For example, undergraduate students who were accepted into the program in a previous year should use the advising packet from that academic year.

It is unlikely that there will be major changes to university and departmental policies, but it is the responsibility of each student to ensure that the requirements are met.
