Online Forms and Documents for Undergraduates

This page provides access to important forms and documents for use by students of the Department of Speech and Hearing Science at Arizona State University. It does not replace academic advising offered to students by staff members of the department. Students are strongly advised to consult their advisors concerning academic affairs and to treat the advising resources available below only as a supplement.

Please be advised that the original hard copies students receive from the Department of Speech and Hearing Science at ASU overrule any online forms you may download, should there be a discrepancy between the official hard copies and the electronic version. Please make certain that the documents obtained are the same as the ones that were in effect at the time you enrolled in the program. For example, undergraduate students who were accepted into the program in a previous year should use the advising packet from that academic year.

It is unlikely that there will be major changes to university and departmental policies, but it is the responsibility of each student to ensure that the requirements are met.
